Booklet Set to open all custodial traditional IRAs. The plan agreement booklet includes beneficiary provisions, disclosure statement, financial disclosure as well as thorough instructions. The application is a 2-part form that includes financial projections on the reverse side.

Purpose of Form: Form 5305-A is a model custodial account agreement that meets the requirements of section 408(a) and has been pre-approved by the IRS. A traditional individual retirement account (traditional IRA) is established after the form is fully executed by both the individual (depositor) and the custodian and must be completed no later than the due date of the individual's income tax return for the tax year (excluding extensions). This account must be created in the United States for the exclusive benefit of the depositor and his or her beneficiaries.

Trust Booklet Set Form# 41-T Revised 10/07

Booklet set to open all traditional trust IRAs.

This set offers the same features as the 40-T and provides the appropriate language for opening a "Trust" IRA plan. (In the case of a trust IRA, the financial projection is used only when the initial traditional IRA investment is one whose IRA investment is one whose earnings can be reasonably projected.) For those institutions choosing to offer trust IRA plans, the CWF # 41-T makes plan opening easy.
